U-M to Hold Open Tryouts

ANN ARBOR, Mich. -- The University of Michigan men's lacrosse team will hold open tryouts for all interested and enrolled male students. The medical requirements for a student looking to try out are as follows:
• Signed WAIVER FORM 
for prospective student-athlete non-rostered walk-on participants
• Pre-participation exam completed by a physician (MD or DO) done within the last 12 months (prefer within the last six months) clearing them for full participation in tryouts
• Copy of front and back of current health insurance card
• Proof of Sickle Cell Trait test OR signed sickle cell trait testing waiver
• U-M ID number
Those interested must also contact administrative assistant Janet Hinz (athinzj@umich.edu) and provide their name and UMID number prior to Sept. 15. You can also call 734-763-3679 to provide the information.
